Hurt
Is your pet's pain managed successfully? Do they breathe easily?

Hunger
Are they eating enough and receiving proper nutrition?

Hydration
Are they drinking and maintaining fluid levels?

Hygiene
Can they be kept clean and dry easily?
"On a scale of 0-10, how easy is it to maintain their hygiene? (10 = Clean, dry, no sores)"
Happiness
Do they still find joy in their favorite things?
"On a scale of 0-10, how happy does your pet seem? (10 = Joyful, responsive, engaged)"
Mobility
Are they able to move with or without assistance?
"On a scale of 0-10, how is their ability to move? (10 = Moving easily, or happy with assistance)"
More Good Days
Are the good days outnumbering the bad?
"On a scale of 0-10, how frequent are the good days? (10 = Nearly all days are good days)"
